What is Balloon Mitral Valvotomy?

Our Heart has four valves, which ensure unidirectional flow of blood across the chambers of the heart.
Mitral valve is one of the four valves which directs flow of blood from left atrium to left ventricle.

A narrowing or restriction of the flow of blood through the mitral valve due to some pathology of valve is called mitral stenosis.

When the mitral valve gets narrowed, it causes the blood vessels to stagnate in the left atrium. This can cause breathlessness, palpitations and chest pain, etc.

Most common in our country is rheumatic affection of mitral valve.

When the stenosis or narrowing becomes significant, then we have to relieve the obstruction which can be done with minimally invasive procedure like balloon mitral valvotomy.

If valve structure is not suitable then we may have to opt for open heart procedure like valve repair or replacement.

Balloon Mitral Valvotomy is the most effective treatment for Mitral Stenosis.

In this process the mitral valve, which is narrowed, is expanded using a balloon to allow blood flow more freely across the heart.

This minimally invasive procedure is often referred to as Percutaneous Balloon Mitral Valvuloplasty (PBMV) or Percutaneous Transvenous Mitral Commissurotomy (PTMC).

A thin, flexible tube (catheter) is introduced through the femoral vein of the groin before being inserted through the heart. Once the catheter has reached the mitral valve that is narrowed, the balloon that is located at the tip will be quickly inflated. This widens Mitral valve leaflets, which are fused and narrowed, and expands the valve opening. This allows for more blood flow through the left atrium to the ventricle of left.

What is balloon mitral valvotomy required to be done?

If the mitral valve area isreduced to less than 1.5 square centimeters. (Normal mitral valve area is 4 -6 square centimeters)

What are the dangers of this procedure? Balloon Mitral Valvotomy?

Balloon mitral valve is a catheter-based that is not surgical. It is less prone to more complications and deaths than mitral valve replacement or repair.

What is the complication of the Balloon Mitral Valvotomy?

Mitral Valve Regurgitation

This could occur in one per 10. Valve could be damaged and it is unable to close properly it allows blood flow into the heart.

Restenosis

The mitral valve typically narrows following 10 to 20 years’ balloon valvotomy. If there is a case of restenosis, the treatment will depend on the state of the valve. It could require a balloon valve or mitral valve replacement surgery.

What are the results after Balloon Mitral Valvotomy?

Short-term outcomes

Relief of symptoms occurs immediately and dramatic following balloon mitral valvotomy

Early(immediate) result for Balloon Mitral Valvotomy result in outstanding

Around, 1.0 to 2.0cm2 increase in the valve’s area, resulting in a decrease in left atrial pressure. This further boosts the heart output.

Long-term outcomes

Improvement in hemodynamics
The resistance to vascularization of the pulmonary tract decreases and can even normalize.
